Review of 725,000 prescription drug monitoring samples demonstrate older patient population — particularly “Boomers” — are at risk for Rx misuse, abuse and drug interactions

BALTIMORE, Md. – May 17 – AmeritoxSM, the nation’s leader in pain medication monitoring, announced today the presentation of a peer-reviewed scientific poster titled “Patterns of Drug Use in the Older Chronic Pain Population” at the American Pain Society Annual Meeting. Ameritox researchers will share analysis from 725,679 urine drug monitoring samples from chronic pain patients aged 50 and older. Data show almost one-third (31.8 percent) lacked the physician-prescribed pain medication in clinically sampled urine, and more than a quarter (28 percent) of patients’ samples contained a prescription drug not prescribed by the doctor ordering the monitoring test.

Although the common perception is that America’s prescription drug misuse concerns should focus on the young, data suggest that older pain patients have the same issues with potential non-adherence and medication misuse as the overall chronic-pain population. The Ameritox study suggests health professionals should put aside age as a predictive indicator in evaluating medication adherence. The study concludes that there is a significant potential for non-adherence in the older population.

“This study provides strong evidence that there are two major reasons to monitor medication use, using urine drug testing, in older patients with chronic pain. The first is that this population has a risk of medication misuse and illicit drug use that warrants attention,” said Dr. Harry Leider, Chief Medical Officer of Ameritox. “The second is that a powerful medication was found in one in four older patients — one that the doctor prescribing a pain medication was not aware of. This data provides a compelling rationale for routinely monitoring medication use in older patients on chronic opioids.”

An additional factor that drives a portion of medication misuse in older patients is the use of multiple medications, prescribed by different physicians, for multiple chronic conditions. This places the older population at a higher risk for dangerous drug-drug interactions. In addition, older patients may not remember or keep track of all of their medications prescribed by various physicians. Common side effects and interactions that can occur as a result include: constipation, mental status changes, depression and falls. Drug monitoring can help provide physicians with a more complete picture of a patient’s medication use prior to a serious event occurring.

The “Patterns of Drug Use in the Older Chronic Pain Population” poster was co-authored by Lisa Burke, PhD, MBA, RN, and Monica Fay, PharmD, MBA, associate medical directors at Ameritox, who examined extensive data of urine drug monitoring results taken from patient samples of those 50 years and older and submitted for analysis to Ameritox from October 1, 2009 to September 30, 2011; a total of 725,679 samples. Results of this testing revealed:

7.6% had an illicit drug detected (e.g., marijuana, cocaine metabolite, heroin metabolite or PCP)

28.1% had a non-prescribed drug detected (e.g., opiates, benzodiazepines, barbiturates, etc.)

31.8% did not have a prescribed drug detected (e.g., a prescribed pain medication)

45.9% of samples had no abnormality found.

These data are similar to findings of a 2009 study1 of more than 900,000 samples taken from individuals across age groups. Therefore, presenters suggest that “age may not be a predominate factor in the likelihood of misuse or abuse of drugs.” Frequently, older patients are perceived as more adherent to their physicians’ prescribed drug regimen. These large-scale urine drug test studies show that monitoring could aid significantly in helping physicians2 identify clinical inconsistencies and should augment patient self-reporting3.

The American Pain Society andAmericanAcademyof Pain Medicine clinical recommendations support pain medication monitoring as a tool to help confirm whether patients adhere to their prescribed drug regimen.

“Older adults are the fastest growing subset of theU.S.population and one of the largest groups of chronic pain patients,” said Lisa Burke, PhD, MBA, RN, Ameritox. “With physicians increasingly on the front lines of patient care and public safety, pain medication monitoring, through urine drug testing, is an important tool to help clinicians ensure their patients – regardless of their age – receive the best possible care and achieve relief from chronic pain.”

The U.S. Department of Health and Human Services Administration on Aging reports the older population — persons 65 years or older — numbered 39.6 million in 2009 (the latest year for which data are available). This age group then represented only 12.9% of theU.S.population, approximately one in every eight Americans. By 2030, there will be about 72.1 million older persons, more than twice their number in 2000.

Innovative Report Helps Clinicians Assess Medication Monitoring Results and Reduce Potential Misuse, Abuse and Diversion of Opioids

BALTIMORE, Md.– May 2, 2012 – AmeritoxSM, the nation’s leader in Pain Medication Monitoring Solutions®, announced the official launch of its new Rx Guardian INSIGHTSM Report, a tool developed by Ameritox to help clinicians quickly assess five critical dimensions of medication adherence in patients with moderate to severe chronic pain.

Chronic pain affects more than 50 million people in the United States. Opioid therapy provides needed relief to many patients, yet these powerful medications can also be misused, abused or diverted from the patients who need them. The U.S. Centers for Disease Control and Prevention (CDC) reportedin November 2011 that prescription painkiller overdoses now kill more than 15,000 Americans every year and that 12 million people in theU.S.reported using these drugs for non-medical reasons. The CDC estimated that non-medical use of narcotics, such as Vicodin, OxyContin, Opana and methadone, cost health insurers as much as $72.5 billion each year.

“Physicians are on the front line of delivering quality care by effectively treating painand protecting society by identifying potential patterns of prescription drug misuse, abuseand diversion.– and at Ameritox, we are committed to providing our physician partners with the most advanced clinical tools to support this critical mission,” said Dr. Harry Leider, Chief Medical Officer at Ameritox. “After listening carefully to physician feedback, we designed the Rx Guardian INSIGHT Report to provide accurate, easy-to-read test results that address the key clinical issues facing physicians with a focus on their top two concerns related to patients on opioid therapy – Is my patient taking any drug of abuse or any prescription medications that I don’t know about? The new INSIGHT Report reflects our ongoing commitment to provide physicians with the advanced tools they need, both in our advanced laboratory services and innovative data analysis tools.”

Developed with substantive input from pain management physicians, the Rx Guardian INSIGHT Report provides a concise summary of the most important elements of a patient’s medication monitoring results. It also helps clinicians assess a patient’s medication adherence over time by showing a chronology of previous Rx Guardian CDSM standard scores – a proprietary methodology offered only by Ameritox.

The Rx Guardian INSIGHTSM Report adds context to results from the Ameritox Rx Guardian CD medication monitoring tool, which uses a dynamic database of more than 1,000 pain patients clinically assessed as adherent and a proprietary normalization algorithm to help physicians assess adherence. The Rx Guardian INSIGHT Report’s five-element summary includes:

1. Drugs of Abuse, identifying recent illicit drug use;
2. Prescription Drugs or Metabolites Detected (but not listed on the lab requisition form), showing medications the physician may not have prescribed;
3. Medication Monitoring Results, showing if the patient has been recently taking the pain medications that are prescribed;
4. Rx Guardian CD℠ Results Standard Scores, comparing the patient’s adjusted urine drug levels to a dynamic reference database of patients clinically assessed as adherent; and
5. Rx Guardian CD℠ Historical Trend, tracking the patient’s Rx Guardian CD℠Standard Scores over time.

“The data we provide to clinicians is critical to the doctor/patient conversation around safe use of powerful medications,” said Leider. “The INSIGHT Report is an important and innovative new tool as it provides physicians with a simple and intuitivesummary of our test results — that they can then use to help reduce misuse, abuse and diversion of these painkillers while enhancing patient care and safety.”

BALTIMORE, May 1, 2012 – Ameritox, the national leader in pain-medication monitoring, calls attention to the Millennium Laboratories statements issued in their April 30, 2012 press release (http://www.prnewswire.com/news-releases/millennium-laboratories-commends-floridas-governor-scott-for-strengthening-regulation-of-clinical-laboratories-149472985.html). The Millennium Laboratories press release suggests Florida Governor Rick Scott’s signing of House Bill 787 supports their position on clinical laboratory operation while disregarding the Governor’s public position. Comparison of the Millennium Laboratories April 30, 2012 press release alongside Governor Scott’s April 27, 2012 letter to Florida Secretary of State Ken Detzner provides a clear example of this discrepancy.

“Millennium’s tactics in the marketplace are not acceptable,” said Ancelmo Lopes, chief executive officer, Ameritox. “It is not our interest to mention other companies in public communications; however, we will not let these statements go unanswered when it comes to something as important as patient well-being.”

Governor Scott’s Official Signing Letter Cites Section 35 Concerns

Though Millennium Laboratories suggests that Governor Scott supports their company’s position specifically, House Bill 787 covers a wide array of health care facilities needs – not just how and where drug testing is conducted. Examination of facts demonstrates Millennium Laboratories does not fully convey the Governor’s on-record concerns. In an April 27, 2012 letter to Secretary Detzner, Governor Scott states:

“While I find most of the bill’s provisions very positive, I do have concerns about the language in Section 35 of the bill amending Section 483.245, Florida Statutes. It is critically important for the state to maintain a strict prohibition against unlawful commissions, bonuses, kickbacks, rebates or split fee arrangements. However, the current interpretation of the anti-kickback law as it relates to the use of trained specimen collectors and the lease of space in the physician offices merits further scrutiny. To that end, I will direct the Secretary of the Agency for Health Care Administration to work with representatives of the clinical lab industry to examine this issue and develop alternative approaches to regulating this particular area of health care law.”

Ameritox is encouraged by the Governor’s letter to Secretary Detzner and, as the national leader in medication monitoring, looks forward to the opportunity to work collaboratively with AHCA and other leading clinical laboratories on this issue crucial to patients suffering from chronic pain.

Full text of Governor Scott’s letter is available via this Florida state government link: http://www.flgov.com/wp-content/uploads/2012/04/4.27.12-HB-787-Transmittal-Letter.pdf

Ameritox Plans Are OIG Reviewed

In the same April 30 Millennium Laboratories press release, Millennium Laboratories uses a veiled attempt to attack Ameritox, writing: “a Maryland-based clinical laboratory that agreed in November 2010 to a $16.3 million settlement.” This statement disregards the historic timeline of events.

In reality, the current Ameritox management team was brought aboard post-2005 events to reach this 2010 Corporate Integrity Agreement between the Office of the Inspector General (OIG) of the US Department of Health and Human Services and Ameritox. Today, under this Corporate Integrity Agreement, Ameritox customer relationship programs are examined by an internal compliance committee, independent external reviewer and the Federal government. Specifically, the Ameritox Specimen Processor customer program is the only one of its kind in the industry that has been reviewed by the OIG and that operates in consultation with the OIG.

Ameritox, as the national leader in pain medication monitoring, believes it is important to provide transparent exchange in both business practice and clinical science. Ameritox adheres to the highest standards in Federal and state policy.

About Ameritox

BALTIMORE, April 20, 2012 – Ameritox, the national leader in pain medication monitoring, today announced that it filed an amended complaint against San Diego-based Millennium Laboratories seeking damages, injunctive and other relief for false advertising, unfair competition, and unfair trade practices. The Ameritox filing alleges that numerous Millennium Laboratories’ business practices have resulted in violations of the Lanham Act, unfair competition in accordance with the Florida Deceptive and Unfair Trade Practices Act, California Business and Professions Code, New Hampshire Consumer Protection Act and national laws relating to unfair competition.

“It is tremendously harmful to physicians, their patients and the industry when a laboratory’s business model, as alleged in this case against Millennium Laboratories, effectively flies in the face of ethical and legal business conduct and the healthcare provider’s decision-making process,” said Dr. Harry Leider, Chief Medical Officer at Ameritox. “These practices of providing financial incentives that mislead health professionals into believing that it is lawful for them to accept illegal inducements are a disservice to the public and the industry. All companies must be held to the same high standards in providing patient-care services.”

Federal Anti-Kickback Statute and numerous state anti-kickback statutes, including those of Arizona, Florida, California, New Hampshire and Texas, prohibit companies from providing financial inducements to health providers in order to induce the referral of business. These laws prohibit direct payments to physicians as an inducement for the referral of business, and prohibit other forms of kickbacks, such as the provision of equipment, supplies, tests, services, and other benefits to the health care provider at no cost or at below fair market value, as alleged against Millennium Laboratories.

Ameritox, as the national leader in pain medication monitoring, believes it is important to provide a transparent exchange of business practices. The Company’s programs are subject to OIG verification and Ameritox adheres to the highest standards in Federal and state policy.

Reporters have online access to complete documentation for this case — Ameritox, LTD. V. Millennium Laboratories, Inc. in the UNITED STATES DISTRICT COURT MIDDLE DISTRICT OF FLORIDA TAMPA DIVISION – via the public access service Public Access to Court Electronic Records (PACER). Case number is 8:11-cv-00775-SCB-TBM.

BALTIMORE, April 19, 2012 /PRNewswire/ — Ameritox is the recognized national leader in pain-medication monitoring owing to its commitment to support science, its long-public national ethical standards and its best-in-class compliance program developed in consultation with the Office of Inspector General (OIG). During the past months, a San Diego-based drug testing laboratory has made comments in press releases about the business practices of Ameritox and other major competitors. Though its longstanding preference has been to not engage, Ameritox has determined that this steady flow of propaganda requires us to correct the inaccuracies issued by this company and to provide accurate information.

“As a leading statesman once said, you’re entitled to your own opinion – you’re not entitled to your own facts. That our San Diego competitor seeks to reinvent history is inappropriate business behavior. Customers rely on accurate and truthful information to make good decisions for patients and anything less is shameful,” said Ancelmo Lopes, chief executive, Ameritox. “We’re proud that Ameritox is the only pain medication monitoring company with a fully implemented, effective compliance program that meets all of the requirements of the Office of Inspector General (OIG) of the US Department of Health and Human Services,” he added.

History of High Standards: Ameritox Plans Are OIG Reviewed

Ameritox works closely with independent and Federal agencies to review its customer support programs, in a review process that represents an industry best practice. The current Ameritox management team was brought aboard post-2005 events, which led to a 2010 Corporate Integrity Agreement between the Office of the Inspector General (OIG) of the US Department of Health and Human Services and Ameritox. Under the Corporate Integrity Agreement, customer relationship programs are examined by an internal compliance committee, independent external reviewer and the Federal government. Specifically, the Ameritox Specimen Processor program is the only one of its kind in the industry that has been reviewed by the OIG and operates in consultation with the OIG.

No Free Goods Provided in Accordance to OIG Rules

Customers rely on their service providers to adhere to the highest standards. Ameritox supports a Physician’s choice to use Point of Care Testing for their patients; however, Ameritox does not provide customers with free or discounted point-of-care (POC) drug-testing cups. Providing gratis or reduced-cost POC cups for physicians to perform billable testing is a questionable practice that Ameritox believes is a violation of anti-kickback laws, therefore putting physician practices at risk.

Ethical Standards are for the Nation — Not State-by-State

Ameritox believes that ethics are concrete and not subject to local competitive whims. Consistent with OIG customer relationship program review, our company will place onsite processors in physician offices to ensure accurate processing of pain medication monitoring paperwork where such processors are permitted. This OIG reviewed program supports ongoing delivery of monitoring in a market with an epidemic of opioid abuse. The San Diego-based company hotly contends this practice in Florida should be stopped and is bolstering this position with PAC-fund support for legislators considering policies against this practice. However, in as many as 15 other states the same company openly supports the very practice it claims to abhor, employing their own specimen processors yet not under OIG oversight. At Ameritox, we believe in one set of values that must not be twisted for competitive advantage. Physician information and patient care are our priorities.

No Need to Retest Negative Drug Samples

Retesting samples screened initially by a laboratory as negative enables companies to charge additional fees for unneeded services — the process neither advances patient care nor controls taxpayer costs. Ameritox, unlike a San Diego company, does not recommend customers retest negative drug samples much less make it a standing practice to retest all negative drug samples.

“For clinicians treating the 50 million US chronic-pain sufferers, helping patients find safe, effective relief is the ultimate priority — and ensuring that medications are taken appropriately is a critical part of that mission,” said Harry Leider, MD, Chief Medical Officer, Ameritox.

Lobbying and PAC Funds

Ameritox invests in science that advances patient pain-medication monitoring and not in political campaigns. Ameritox believes that by advancing the science and providing clinical support for pain-medication studies, experts will be able to determine how and when to best use these tests to complement Federal government policies. That said, its San Diego competitor supports state-government legislative efforts and funds those opinions with PAC resources. In Florida alone, where pending key legislation benefits that company, it has targeted $20,000 in contributions.

Ameritox, as the national leader in pain medication monitoring, believes it is important to provide transparent exchange whether business practice or clinical science. The Company’s client relationship programs are subject to independent audit and OIG review and Ameritox adheres to the highest standards in Federal and state policy.

Shared Mission to Reduce Misuse, Abuse and Diversion of Pain Medication Critical to Patient Care and Public Health

BALTIMORE, MD. (APRIL 6, 2012)—AmeritoxSM, the nation’s leader in pain medication monitoring, commends Unlawful Narcotics Investigations, Treatment and Education, Inc. (Operation UNITE), the organizers of the inaugural National Rx Drug Abuse Summit, on its coordination of a national forum to address the prescription drug abuse epidemic. The Summit will be held April 10-12 in Orlando, Florida.

“As the nation’s leader in Pain Medication Monitoring Solutions®, Ameritox, like Operation UNITE, is committed to understanding the reasons behind the prescription drug abuse epidemic and working with state and Federal advocates toward a rapid and effective solution,” said Dr. Harry Leider, Chief Medical Officer, Ameritox. “In our industry leadership role, Ameritox is charged with providing physicians who prescribe opioid therapy with effective tools, supported by quality research, to help ensure that proper medications are getting to the millions of chronic pain patients who need it. In this way, we support physicians who treat patients with chronic pain and their efforts to reduce the misuse, abuse and diversion of prescription opioid medication.”

Medication monitoring through urine drug testing is a critical tool in helping physicians provide the best care for patients receiving long term treatment with opioids. In February, Ameritox announced its support of a new set of expert consensus recommendations designed to bring uniformity to the use of pain medication monitoring and add specific recommendations to previous clinical guidelines. The recommendations also provided guidance on how to use urine drug monitoring to improve patient outcomes.

The leading service offered by Ameritox, Rx Guardian CDSM, is a proprietary monitoring tool available to clinicians who prescribe opioids to patients with chronic pain aimed at helping to reduce the risk of opioid misuse, abuse and diversion. Rx Guardian CD consists of a reference database of more than 1,000 pain patients who were clinically assessed as adherent to their prescription regimens. It then displays the results in an innovative format, allowing physicians to see, at a glance, how a specific patient’s results compare to patients in the database.

Ameritox recently launched the Rx Guardian INSIGHTSM Report, a tool developed with input from pain management physiciansand designed to provide a concise summary of the most important elements of a patient’s medication monitoring results. The report helps clinicians rapidly assess a patient’s medication monitoring results over time by showing a chronology of previous Rx Guardian CD results.

“The number one cause of accidental death in the country is drug overdose, and prescription drug overdoses account for a significant number of these avoidable tragedies,” Dr. Leider said. “The prescription drug abuse epidemic is the driving force behind the Ameritox commitment to work closely with physicians who prescribe these powerful medications. We recognize that physicians are on the front lines of treating pain and reducing prescription drug misuse, abuse and diversion.”

Collaboration Prepares MDDScoreTM Test Commercialization for Primary Care and Pain Medicine Physicians

Assay analyzing multiple biomarkers could be valuable aid to diagnosis, treatment monitoring

SAN DIEGO, CA, and BALTIMORE, MD, APRIL 5, 2012 – Ridge DiagnosticsTM, Inc., a neurodiagnostic company, announced today it has entered into an exclusive agreement with AmeritoxSM, Ltd., a leader in medication monitoring, for the license rights to its proprietary developmental blood test MDDScoreTM, a multiple biomarker analysis for Major Depressive Disorder (MDD) for primary care and pain physicians. As part of the agreement, Ameritox has agreed to make an undisclosed equity investment in Ridge and commit to a next-stage clinical development program.

Antidepressant drugs are among the most frequently prescribed medications in the U.S. today, where depression impacts some 19 million adults each year. A biological test for depression may aid physicians in the differential diagnosis of MDD, assist in the selection of proper patient treatment and improve healthcare cost management.

“Ameritox investment in Ridge will likely accelerate the MDDScore test development and advance access to millions of patients already under the care of primary-care and pain-management physicians,” said Lonna J. Williams, CEO of Ridge Diagnostics. “Ameritox longstanding investments in diagnostic science, and now in the Ridge technology, validate the importance of the MDDScore and demonstrate their commitment to improving patient well-being.”

The MDDScore test developed by Ridge Diagnostics measures ten biomarker levels associated with factors such as inflammation, development and maintenance of neurons and interaction between brain structures involved with stress response and other key biological functions. Measurements are analyzed to produce an MDDScore which may help physicians determine whether a patient has major depression.

A recent peer-reviewed study with 70 adults who had been diagnosed with major depression evaluated in a pilot and replication study using the MDDScore appears in Molecular Psychiatry (February 2012) and indicates the test diagnosed major depression with a high degree of accuracy within the study population. The study is co-authored by investigators from Massachusetts General Hospital, Boston, MA; Vanderbilt University, Nashville, TN; Cambridge Health Alliance, Boston, MA, and Elizabeth Medical Center, Brighton, MA.

“Biomarkers are an important emerging scientific tool to better understanding illness, and may enable health professionals to improve the accuracy of diagnosis and help match the right treatment to the right patient,” said Ancelmo Lopes, CEO, Ameritox Ltd. “Personalizing care offers the best use for precious healthcare resources. We are committed to providing physicians with tools to support their care decisions. We are excited to launch clinical trials to evaluate the utility of MDDScore in primary care and pain medicine settings.”

If adopted, Recommendations Could Help Improve Adherence and Reduce Misuse, Abuse and Diversion of Powerful Painkillers

BALTIMORE, MD (February 26, 2012)—AmeritoxSM, the nation’s leader in pain medication monitoring, commends the expert authors of “Recommendations For Urine Drug Monitoring For Patients On Long-Term Opioid Therapy,” unveiled Friday evening in a poster session at the American Academy of Pain Medicine’s (AAPM) 28th Annual Meeting in Palm Springs, California. The expert consensus recommendations seek to answer key questions that physicians may have about medication monitoring for patients prescribed powerful painkillers for extended periods.

The recommendations are an effort to bring uniformity to the use of pain medication monitoring, a tool that many physicians use to aid treatment decisions for patients on chronic opioid therapy. Medication monitoring is an important means to help physicians staunch the growing number of prescription painkiller overdoses. In a report late last year, the Centers for Disease Control put the number of annual overdoses from narcotic pain relievers such as Vicodin, Oxycontin, Opana and methadone at more than 20,000. The CDC called it a “public health epidemic.”

“Advances in lab-based technology like urine drug monitoring, or UDM, represent critical tools that inform and support clinical decision making,” said Joseph E. Couto, PharmD, MBA, of the Jefferson School of Population Health in Philadelphia and leader of the panel discussions. “These consensus recommendations are intended to provide guidance around the appropriate use of UDM in clinical practice – and we hope they will lead to the eventual development of formal guidelines.”

The recommendations spell out how a physician treating chronic pain patients can determine which patients to test, how frequently to monitor, and what clinicians should do in response to abnormal test results. For instance, the consensus calls for drug monitoring a patient with medium to high risk of misuse at least four times a year.

Urine drug monitoring allows physicians to monitor the presence of pain medication in a patient’s system, helping them to ensure the patient receives adequate medication to provide relief, while also helping to prevent potential misuse, abuse or diversion of powerful narcotics.

Consensus Working Group members include respected practitioners and national authorities in the treatment of chronic pain: John F. Peppin, DO, FACP; Perry G. Fine, MD; Steven D. Passik, Ph.D.; and Charles Argoff, MD

The development of the recommendations was funded by Ameritox, a Baltimore-based Pain Medication Monitoring Solutions® company, which provided an unrestricted grant to the Jefferson School of Population Health.

“UDM is an indispensable component of chronic pain treatment today, yet primary care physicians and pain specialists have been practicing without the benefit of consensus or best-practice national standards,” Dr. Harry Leider, Chief Medical Officer of Ameritox. “We provided this grant and then stood aside to let this distinguished panel of experts propose what made the most sense for both physicians and their patients.”

BALTIMORE, MD. (February 23, 2012)—AmeritoxSM, the nation’s leader in pain medication monitoring, will sponsor a safe opioid prescribing symposium highlighting emerging trends and technologies in medication monitoring at the American Academy of Pain Medicine’s (AAPM) 28th Annual Meeting in Palm Springs, California.

“For clinicians treating the tens of millions of people suffering from chronic pain, helping patients find safe, effective relief is the ultimate priority,” said Dr. Harry Leider, Chief Medical Officer at Ameritox.

The symposium will focus on a new set of expert consensus recommendations, to be presented during a poster session at the meeting, which answer critical questions in pain monitoring. These recommendations are intended to add specifics to previous clinical guidelines, with the goal of providing physicians guidance on how to use urine drug monitoring to improve patient outcomes.

The symposium will also provide attendees with the latest data on the emerging use of urine opioid levels in helping physicians assess patients for adherence to a chronic opioid regimen. This segment will highlight Ameritox research, which resulted in a proprietary monitoring tool, Rx Guardian CDSM, being made available to clinicians who prescribe opioids to patients with chronic pain to help reduce the risk of opioid misuse, abuse and diversion. Rx Guardian CDSM consists of a reference database of more than 1,000 pain patients who were clinically assessed as adherent to their prescription regimens. It then displays the results to allow physicians to see, at a glance, how a specific patient’s drug monitoring results compare to patients in the database.

“Physicians are on the front lines of both patient care and drug abuse prevention,” Leider said. “Providing tools, supported by quality research, to help physicians evaluate whether their patients take the proper medications as directed should improve patient care and help keep prescription painkillers out of the hands of those who abuse these medications.”

Topics and Faculty include:

BALTIMORE, Md. – Nov. 11, 2011 – AmeritoxSM announced today that it has joined Hero Health Hire, a coalition of companies and other entities in the health care industry that are united with the goal of employing disabled veterans. Launched in Washington, D.C., at an industry summit that included the U.S. Secretary of Labor, members of Congress, and representatives of a number of government agencies and the military, Hero Health Hire includes companies, associations, and hospitals from across the spectrum of health care that collectively employ thousands of employees. Each has committed to working to help veterans find and retain jobs in health care.

“Ameritox is proud to be part of this important, united effort to help disabled veterans secure meaningful job opportunities,” said David Mack, vice president of human resources at Ameritox, the nation’s leader in Pain Medication Monitoring Solutions ®. “Veterans have a lot to offer Ameritox and other companies – they are dedicated workers, with unique backgrounds and skill sets.”

Each month, tens of thousands of “Wounded Warriors” – brave men and women in our military who have been injured or become ill during service – transition out of the military only to struggle as disabled veterans in the civilian workforce. For many, they may be interviewing for a job or writing a resume for the first time, unsure of how military skills and experiences might translate or what accommodations may be needed for transition to a civilian workplace.

The health care industry – including insurers, health plans, pharmaceutical companies, device manufacturers and hospital networks – is considered the fastest growing industry in our economy, requiring talented individuals to help meet multi-generational health care needs. The industry is also uniquely positioned to understand and support the needs of our nation’s Wounded Warriors.

Hero Health Hire unites the health care industry with government agencies and the military to understand and eliminate the barriers to employment facing Wounded Warriors, as well as developing ways to the support them in their transition.

Retired Maj. Gen. Elder Granger joins Hero Health Hire

The Hero Health Hire initiative also recently announced that retired Maj. Gen. Elder Granger, M.D., has been named honorary chairman of the initiative. Granger, currently co-founder and president of The 5Ps LLC, served the military for 37 years, most recently as deputy director of TRICARE Management Activity for the Office of the Assistant Secretary of Defense for Health Affairs. In his role, Granger will provide valuable insights as partner companies establish their internal programs, serve as a liaison to the military community to help raise awareness about the program, and provide additional support as the program expands.

“I am honored to join the Hero Health Hire initiative and help our nation’s disabled veterans and all veterans find meaningful employment opportunities with America’s leading health care businesses,” said Granger. “Hero Health Hire comes at a critical time for our veterans, and I look forward to working with these businesses to recruit more disabled veterans and assist them in their transition.”

BALTIMORE – Oct. 27, 2011 – AmeritoxSM, the nation’s leader in Pain Medication Monitoring Solutions®, earlier this month submitted a Statement for the Record for the Congressional hearing on “Costs of Prescription Drug Abuse in the Medicare Part D Program,” pointing to medication monitoring as a potential solution to improving adherence to pain medication, preventing drug diversion and aiding in lowering national healthcare costs. The hearing was held by the United States Senate Committee on Homeland Security and Government Affairs Subcommittee on Federal Financial Management, Government Information, Federal Services and International Security.

“As the nation’s leader in Pain Medication Monitoring Solutions, Ameritox is committed to helping state and Federal policymakers understand the reasons behind rising prescription drug abuse, and identifying proven and cost-effective solutions,” said Dr. Harry Leider, chief medical officer, Ameritox. “Medication monitoring is a critical tool in helping physicians provide the best possible care for patients. This simple, scientific tool can help assure doctors that the proper medications are getting to the millions of chronic pain patients who need it, and not contributing to misuse, abuse and diversion of prescription opioid therapy, costing payers millions of dollars each year.”

Multiple studies have shown that medication monitoring, along with improving patient care, has the added benefit of reducing healthcare costs. One study published in The American Journal of Managed Care in January found that patients on chronic opioid therapy who were likely non-adherent, as determined by urine drug testing results, had 14 percent higher health care costs and 35 percent more hospital days. The same study found that patients who may be adherent to their opioid regimen had annual costs that were approximately 12 percent lower than non-adherent patients.

Dr. David Nash, dean of the Jefferson School of Population Health at Thomas Jefferson University in Philadelphia, PA, said such studies demonstrate that drug monitoring that uses advanced laboratory technologies is becoming a standard of care because it is a key component to quality care for patients on long-term opioid therapy.

“Addressing chronic pain and improving adherence to opioid therapy presents us with a major opportunity to raise the quality of care and reduce healthcare costs at the same time,” Nash said.

More people die each year in the U.S. from prescription opioid overdose than from cocaine and heroin use combined

Traditional urine drug testing, while important, has limitations with respect to distinguishing adherence versus potential opioid abuse or diversion

Conducted at Beth Israel Medical Center in New York and presented at PAINWeek annual meeting in Las Vegas, the study showed that a large database of adherent patients can serve as a reference to help evaluate potential opioid use across diverse patient populations – providing clinicians with an important tool to improve patient care through pain medication monitoring

BALTIMORE, MD (September 9, 2011)—AmeritoxSM, the nation’s leader in pain medication monitoring, in conjunction with Beth Israel Medical Center, this week announced the results of a new study showing that the company’s reference database tool, Rx Guardian CDSM, based on urine drug monitoring results from a large database of adherent oxycodone-prescribed patients, generalized well to a geographically and demographically distinct group of patients. Study findings were presented today at PAINWeek®, the national conference on pain for frontline practitioners, the nation’s largest pain conference.

Results of this study indicate that Rx Guardian CDSM, when combined with other clinical indicators of non-adherence, should enhance clinicians’ ability to help identify potential non-adherent patients using chronic opioid therapy.

Russell Portenoy, MD, Chairman of the Department of Pain Medicine and Palliative Medicine at Beth Israel Medical Center in New York, was principal investigator of this study sponsored by Ameritox. The purpose of the study was to determine whether the proprietary model, based on a database of adherent patients from Wisconsin, would generalize well to a different population of adherent, oxycodone-treated patients from New York City. Study results confirmed that adherence reference ranges calculated from a Wisconsin patient sample generalized well to the New York City sample, further demonstrating the applicability of the tool across diverse patient populations.

“Prescription opioid therapy is a critical lifeline for millions of patients suffering with chronic pain, but careful monitoring is important for both individual and public safety – in addition to health concerns, misuse, abuse and diversion of prescription opioid therapy costs payers millions,” said Dr. Harry Leider, a contributing author of the study and Chief Medical Officer of Ameritox. “By developing tools based on new drug monitoring technologies that can help physicians better detect non-adherence, we are contributing to improved quality of care and potentially reducing costs to the healthcare system.”

In this era of health care reform, reduction of unnecessary medical spending is an important goal. A major 2011 study published in The American Journal of Managed Care demonstrated that patients on chronic opioid therapy who were likely non-adherent, as determined by urine drug monitoring results, spent 35 percent more time in the hospital and had 14 percent higher health care costs than patients who were likely adherent. A separate study, published this year in The American Journal of Pharmacy Benefits, found that non-adherent patients had average excess medical costs of $20,546 per year, per privately insured patient, when compared to likely adherent patients.

While most patients and physicians use or prescribe chronic opioid therapy with the best of intentions, a recent national study of nearly one million prescription monitoring tests showed that, for a variety of reasons, the majority of pain patients may not be using their medication correctly – indicating potential misuse, abuse, diversion or non-adherence. For example, 11 percent of patients were found to be taking an illicit drug and more than 30 percent had no detectable level of their prescribed medications – despite being prescribed a chronic regimen of opioid therapy.

Study Methodology

The purpose of the study was to determine whether the Rx Guardian CDSM model, based on a database of adherent patients from Wisconsin, would generalize well to a population of adherent, oxycodone-treated patients from New York City. A cohort of pain patients receiving a stable regimen of oxycodone therapy was identified from a larger study at New York’s Beth Israel Medical Center (BIMC). Investigators established adherence to opioid therapy using multiple criteria including: appropriate self-reported medication use, the Morisky Validity of Medication Adherence survey, and urine drug testing results demonstrating the absence of illicit drugs or non-prescribed controlled medications, and the presence of the prescribed opioid. Quantitative urine oxycodone levels, measured with mass spectrometry, were obtained and adjusted for hydration and lean body weight using the proprietary algorithm. These adjusted urine drug levels were compared to reference ranges derived from a pre-existing database of patients, who were rigorously assessed to be adherent by a pain medicine program in Wisconsin. Study investigators computed the percentage of patients classified as adherent by BIMC that also fell within the reference ranges (with corresponding 95% binomial confidence intervals). Study investigators reported these values separately for patients on variable or fixed dosing regimens.

Results

In total, 108 BIMC patients receiving chronic oxycodone therapy provided 112 urine samples; 60 samples were from patients taking a fixed total daily dose of oxycodone and 52 were obtained from patients on flexible but stable doses.

These urine drug levels were adjusted for hydration and lean body weight using the proprietary algorithm and were then compared to the pre-established reference ranges derived from the Wisconsin cohort of adherent patients. Of the total 112 BIMC samples from known adherent patients, 99 or 88.4% (95% CI: 81%, 93.7%) were found to be within range. For patients on fixed dose oxycodone, 54 of 60 or 90% (95% CI: 79.5%, 96.2%) were found to be within range; for those on variable dose oxycodone, 45 of 52 or 86.5% (95% CI: 74.2%, 94.4%) were found to be within range.

Study Implications

Chronic pain is the most common cause of long-term disability in the United States, affecting 50 million Americans. Treatment with prescription narcotics (i.e., opioids) is an effective and critical treatment for a significant subset of this population with severe pain that cannot be controlled with other medications and treatments. However, opioid misuse, abuse, diversion and non-adherence is a major threat to patient safety and impacts negatively total healthcare spending. Traditional drug testing approaches, such as urine drug testing alone, cannot distinguish between patients taking their opioid regimen correctly from patients taking too little opioid (potential non-adherence or diversion) or taking too much opioid (potential misuse or abuse). This study demonstrated that the proprietary Rx Guardian CD tool, based on a large database of adherent patients in Wisconsin, can be applied to diverse patient populations across the country to help identify potential misuse, abuse or diversion of opioid therapy – thereby providing clinicians with an important tool to improve patient care and potentially impact medical costs. Study results are available online at www.ameritox.com/research.

BALTIMORE, Md. – July 20, 2011 — AmeritoxSM announced today that the Accreditation Committee of the College of American Pathologists (CAP) has awarded accreditation to Ameritox’s laboratories in both Greensboro, N.C., and Midland, Texas.

The accreditations were based on the results of recent onsite inspections of the laboratories, which now join more than 7,000 CAP-accredited laboratories worldwide. The federal government recognizes the CAP Laboratory Accreditation Program, which began in the early 1960s, as being equal to or more stringent than the government’s own inspection program.

“We were delighted to work with an organization like CAP, because their standards are extremely rigorous and they objectively examined every aspect of our laboratories,” said Jay Zimmerman, Chief Operating Officer at Ameritox, the leader in Pain Medication Monitoring Solutions®. “By meeting CAP’s standards, we have shown our clients that we are focused on laboratory service excellence.”

During the CAP accreditation process, inspectors examine laboratory records and quality control of procedures for the preceding two years. CAP inspectors also examine laboratory staff qualifications, as well as the laboratory’s equipment, facilities, safety program and record, in addition to the overall laboratory management. This stringent inspection program is designed to specifically ensure the highest standard of care for all laboratory patients.

“By working with CAP,” Zimmerman said, “we continue to affirm to our customers that every testing method in our laboratories has been scrutinized, every person working in our laboratories is thoroughly trained and schooled in the proper procedures, and every piece of equipment is performing optimally.

“We understand that physicians rely on the quality and accuracy of the tests that our laboratories perform,” he said, “and that those test results can help a doctor better monitor their patients’ health care.”

About CAP

The College of American Pathologists is a medical society serving more than 17,000 physician members and the laboratory community throughout the world. It is the world’s largest association composed exclusively of board-certified pathologists and is widely considered the leader in laboratory quality assurance. The College is an advocate for high-quality and cost-effective patient care.

The CAP Certification Mark is a service mark owned by CAP and is used pursuant to a license from CAP.

BALTIMORE, MD (June 9, 2011) —Ameritox, the leader in pain medication monitoring, today announced the launch of Rx Guardian CDSM, its next-generation pain medication monitoring solution and part of the patented Rx GuardianSM process. This new science will help physicians interpret patients’ urine drug test (UDT) results so they can identify potential misuse, abuse or diversion of opioid pain medications. A propriety algorithm and reference database of more than 1,000 chronic pain patients assessed for adherence to their prescription regimens, Rx Guardian CDSM will assist clinicians as they evaluate if patients are taking medications as prescribed.

“For clinicians treating the 50 million U.S. chronic-pain sufferers, helping patients find safe, effective relief is the ultimate priority – and ensuring that medications are taken appropriately is a critical part of that mission,” said Harry Leider, MD, Chief Medical Officer, Ameritox. “The Rx Guardian process has evolved to include the Rx Guardian CD science, offering opioid prescribers a powerful clinical tool to help them assess patient adherence, protect patient safety and improve standards of care.”

Chronic pain is the most common cause of long-term disability in the United States, affecting 50 million Americans2. Treatment with prescription narcotics (i.e., opioids) is an effective and critical treatment for a significant subset of this population that has more severe pain that cannot be controlled with other medications and treatments. However, opioid abuse is a rapidly growing national problem, with substance abuse treatment admissions for opioid abuse among the 18-24 year-old population increasing more than 700% between 1998 and 20083.

To reduce the potential for misuse, abuse and diversion, clinical guidelines created by the American Pain Society and American Academy of Pain Medicine support routine monitoring, including urine drug testing, of chronic opioid patients to help confirm medication adherence. However, a recent study reported that just 8% of patients on long-term opioid treatment receive pain medication monitoring through urine drug testing in primary care practices4.

Rx Guardian CD consists of a new reference database of more than 1,000 pain patients who have been rigorously assessed and found to adhere to their prescription regimens. Doctors can compare their patients’ urine drug test results to those of the adherent pain patients in the reference database, helping to identify patients who are likely taking their medication correctly.

“More than ever, prescribers face the challenge of providing optimal patient care while preventing prescription drug abuse,” said Michael Barnes, spokesperson for CLAAD, the Center for Legal Access and Abuse Deterrence. “Routine monitoring using advanced drug testing technologies can help prescribers improve patient care, identify individuals who need addiction treatment, and deter and detect individuals who intend to divert or abuse medications.”

Patients, who may have no intent to abuse a medication, may not always take them correctly: they may choose to take too little due to fear of addiction, or take too much because of inadequate pain control. While traditional urine drug testing can only report the absence or presence of a drug or metabolite, the Rx Guardian CD process uses an additional technology to assist doctors in determining potential misuse or abuse of opioids. Patient results are adjusted based on physiological variables such as weight and hydration, and these adjusted values are then compared to the reference database, providing clinicians with additional information in assessing pain medication adherence.

BALTIMORE – March 17, 2011 – Patients undergoing opioid therapy for chronic pain who may not be following their prescription regimen have significantly higher overall healthcare costs, Ameritox Ltd. announced recently. The study was published in The American Journal of Managed Care.

“Considering all of the discussion right now around health care reform and controlling health care spending, this study highlights an opportunity to better manage a costly and at-risk population of patients – those on chronic opioid therapy,” said Harry L. Leider, MD, co-author of the study. Dr. Leider is Chief Medical Officer at Ameritox – a corporate sponsor of the research –and a faculty member with the American College of Physician Executives. “By focusing on patients who receive opioids and are likely non-adherent, clinicians can take concrete steps to improve patient safety and, ultimately, lower costs.”

The study analyzed managed care claims of more than 18 million patients, identifying a subset of pain patients on chronic opioid therapy. The study determined that patients on chronic opioid therapy who were likely non-adherent, as determined by urine drug testing results, had 14 percent higher health care costs and 35 percent more hospital days.

Likely non-adherence to prescribed opioid therapy was identified through urine drug monitoring results and included patients with results of a prescribed medication not found, the presence of a non-prescribed drug, the presence of an illicit drug found in the urine, or based on the expected amount of a prescribed drug found.

The study concluded that improving adherence among patients with chronic pain could help reduce health care spending and improve health outcomes. “Opioids are an important tool in pain management,” added Leider. “This study reinforces a growing belief that pain medication monitoring must be part of the pain management paradigm. Urine drug monitoring can provide clinicians with information to help improve medication adherence and outcomes among their patients, while controlling overall costs.”

Among the Key Findings in the Study:

• Overall healthcare spending for patients on chronic opioid therapy was over $23,000 per year (2008 dollars)
• Total health care costs were 14 percent higher for likely non-adherent patients ($26,433) over a 12 month period following initial urine drug testing.
• Likely non-adherent patients had 35 percent more hospital days.

About the Study: Data for the study were obtained from a managed care claims database of 18 million people enrolled between July 1, 2005 and Sept. 30, 2008. The data include geographically diverse commercial, Medicare Advantage and Medicaid health plan members in the United States. Data for classification were obtained from an independent database of urine drug testing results. Patients with evidence of long-term prescription opioid use (defined as at least 120 days over any consecutive six month period) were selected for the study. More than 2,100 patients were classified based on urine drug testing results. The study was funded by Ameritox.

Baltimore, MD (February 4, 2011) —Ameritox, the nation’s leader in pain medication monitoring, made significant strides to advance its industry leadership position by strengthening the company’s commitment to physicians and patients in the past twelve months. Through significant investments in clinical research and an expansion of its laboratory capabilities, Ameritox has reached several milestones – including the analysis of nearly 2.5 million patient samples with its proprietary RxGuardian offering to healthcare partners.

“We’re proud to have reached these milestones through the work of dedicated staff in our Greensboro (NC) and Midland (TX) labs and Baltimore (MD) headquarters, expanding our science-based technology, enhancing service to our physician partners and investing in meeting our customers’ needs,” said Ancelmo Lopes, CEO of Ameritox.

Clinical and Research Highlights

Ameritox, long-known for its service and partnership with healthcare providers, leads the industry with an innovative research program to re-define the field of pain medication monitoring. The Baltimore-based company is the largest laboratory focused exclusively on supporting clinicians who treat patients with chronic pain. Significant research and clinical advances have expanded the tools Ameritox offers to its physician partners, and the corporation’s research program continues to lead the industry.

By adding to the body of knowledge regarding managing patients on chronic opioid therapy, Ameritox continues to assist physicians in making better treatment decisions. In the past two years, nine key studies have been accepted for or presented at national clinical meetings. Four studies have also been published in respected peer-reviewed journals.

In 2010, Ameritox presented three studies at the American Academy of Pain Management meeting (AAPM). A landmark study published in the Journal of Opioid Management demonstrated how the proprietary Ameritox Rx Guardian algorithm, when applied to urine drug levels of OxyContin® C-II (oxycodone HCL controlled-release) tablets, differentiated among three different daily doses of the drug, thus surpassing traditional urine testing. A second study, published in January in The Journal of Clinical Pharmacy and Therapeutics, described Rx Guardian’s efficacy in discriminating between all three daily doses of hydrocodone tested (20, 60, 120 mg). An important health economics study also published in January in the American Journal of Managed Care, “Chronic Opioid Adherence and Healthcare Costs in a Managed Care Population,” linked opioid prescription non-adherence with significantly elevated health care costs.
